Informed consent
Beste patiënten, in deze gedigitaliseerde tijden is het mogelijk voor ons als huisarts om efficiënter samen te werken met andere zorgverleners in de eerste lijn en met de ziekenhuizen. Dit kan alleen door uw geïnformeerde toestemming (informed consent) te geven om uw gezondheidsgegevens te delen.
Wat houdt dit precies in? Als u uw goedkeuring geeft, kunnen zorgverleners relevante gegevens met betrekking tot uw medische voorgeschiedenis, medicatie, vaccinaties, enz. op een beveiligde manier met elkaar delen. Op die manier kunnen we beter samenwerken, onnodige onderzoeken vermijden en u beter behandelen. Enkel zorgverleners waar u een therapeutische relatie mee heeft, kunnen deze gegevens inkijken. Hiervoor wordt telkens ingelogd met uw elektronische identiteitskaart.
Een voorbeeld: u gaat naar de kust en krijgt een ongeval waarvoor u opgenomen moet worden in het plaatselijk ziekenhuis. Door uw toestemming te geven kan de spoedarts nakijken of en welke medicatie u neemt, of u allergieën heeft, of u al eerdere operaties gehad heeft, enz. Hierdoor kan er snel en efficiënter gehandeld worden.
Hoe kan u uw toestemming geven? Dat kan u thuis zelf doen via: www.patientconsent.be. U dient dan in te loggen met uw elektronische identiteitskaart. U kan dit ook bij de huisarts doen, de apotheek of in het ziekenhuis. U kan op elk moment uw toestemming intrekken en u kan steeds vragen aan uw zorgverlener om bepaalde informatie niet te delen.

Global Medical Record (Globaal Medisch Dossier)
What is the GMD?
Your GP collects all your important medical data (operations, chronic diseases, reports from specialists and other health care providers, information about treatments, etc.) in your Global Medical Record (GMD).
The GMD is free for everyone. The annual contribution is fully reimbursed by your health insurance fund. For administrative reasons, the GP arranges this via the direct settlement with your health insurance company. For this purpose, you must bring a health insurance vignette and your identity card.
What are the advantages?
The collection of your medical data allows for better individual support and better communication between care providers. This file ensures that your GP has an overall picture of your state of health. In this way, he/she can better assess which examinations are necessary or unnecessary and whether proposed treatments do not conflict with each other.
If you have a GMD, you will also be entitled to a 30% reduction on the patient fee (i.e. the amount not reimbursed by the health insurance organisation) for a consultation or home visit. The reimbursement rate depends on the category to which you belong (less than 10 years old, chronically ill, palliative patient, etc.).
What is the GMD+?
The GMD+ is the prevention module of the GMD and is free for patients aged between 45 and 75. Once a year, your GP will discuss with you, on the basis of a checklist, which preventive health measures could be useful for you.
How to apply for a GMD or GMD+?
At your next consultation in the cabinet or during a home visit, you personally ask your GP to open a GMD. Parents request the opening of a GMD for their child(ren). The GMD must be renewed annually.
From 1 January of the year you turn 45 until 31 December of the year you turn 75, you can ask your GP for a GMD+. This can be done together with an application or renewal of the GMD, but also during another consultation or home visit.
Are you consulting another doctor?
If you consult a different GP than the one with whom you concluded your GMD, you can benefit from the same rights if these GPs work together in a group registered by the RIZIV (these are both group practices and other collaborative arrangements between GPs).
Specialists are not obliged to pass on data to the GP. When you consult another doctor (general practitioner or specialist), always ask for useful information to be passed on to your GP.
If you move or change GPs, the file can, at your request, be passed on to a new GP.
